After almost 25 years, it is with mixed emotions that I am announcing that I will be leaving private practice and closing my medical office. It has been a pleasure and an honor to have been a part of your healthcare team.

As it is vital for you to continue your routine follow ups, I will continue to be available for your care until August 31, 2024. Starting September 1st, I have arranged with Nephrology Medical Care PLLC to be custodian of your medical records and if you choose, to become your new Nephrologists. They are a local group of physicians who will be able to care for you with great skill and empathy going forward.

All medical records will be held by: Nephrology Medical Care PLLC.  You must contact them for a Records Release Form.  Once signed and returned to them, you may request any of the following:

1) A copy of your medical chart
2) Have it sent to a new physician of your choice
3) An appointment for a new patient visit with them.
